2023-2026 Community Health Improvement Plan (CHIP)

The Allen County CHIP defines where we would like to be as a community, along with our plans to make it happen. We are working on four areas to improve the conditions for everyone to be healthy:

  • Mental Health and Substance Use
  • Supports for Healthy Living
  • Maternal and Infant Health
  • Thriving Communities
